What is metabolic osteoarthritis?
A short overview of how researchers now think about osteoarthritis (OA) — and why "wear and tear" is only part of the picture.
The old picture: a joint wearing down
For a long time, osteoarthritis was described mainly as a mechanical problem: cartilage, the smooth cushioning tissue at the ends of your bones, physically wearing away over years of use — like the tread on a tire. That picture isn't wrong, exactly. Joint mechanics, past injuries, and repeated stress do matter.
But it's incomplete. It doesn't explain why OA can show up in joints that haven't seen unusual wear, why it's linked to conditions like type 2 diabetes and obesity in ways that go beyond extra weight on a joint, or why two people with similarly "worn" joints can have very different symptoms.
The fuller picture: a metabolic process too
Cartilage isn't an inert cushion. It's living tissue, maintained by cells called chondrocytes that are constantly sensing their environment and rebuilding the tissue around them. That maintenance process is metabolic — it's affected by things like blood sugar, fat tissue signaling, and low-grade inflammation circulating through the body, not just by mechanical load.
Researchers increasingly describe osteoarthritis as a "whole joint," metabolically-influenced condition: mechanics still matter, but so does the biological environment those cells are working in. That's the "beyond" in Beyond Wear and Tear — not a replacement for the mechanical story, an addition to it.
